Timely and accurate information is vital for you and your plan members in an ever-changing environment. Fiduciary Services provides you various tools to help keep you and your employees informed.

We will coordinate with your Human Resources Department on the best way to organize and conduct education and enrollment meetings.

• Inform participants about the characteristics, plan design and benefits of your plan.
• Provide basic retirement planning and investment education, focusing on the importance of diversification, investment objectives, historical rate of return and other details of the investment funds selected by your sponsor.¹
• Explain the various disbursement options available during active service or upon termination of employment, including restrictions and tax implications for each payment option.
• Educate on how to manage the account online at popular.com/401k and/or through the voice response system.

While you remain a plan participant, there will be events or transactions defined in your plan document for which your employer will require you to complete certain forms. Most of these forms must be signed and returned to your employer. For your convenience, and in order to increase your plan's administrative efficiency, we have included the following employee forms:

• Enrollment & Changes to the Popular Master Plan Form – To enroll in the plan for the first time, make any changes in your personal information or deferral.
• Declaration of Lost Monetary Instrument Form – To request a new distribution check if the one you received is lost.
• Beneficiary Designation Form – To establish who will receive the plan benefits in case of a death.
• Rollover Form – To instruct Banco Popular to transfer the account balance to another qualified plan in the event of termination.
• Plan Distribution Form: Employment Termination – To instruct Banco Popular to pay out your account balance in the event of employment termination.
• Plan Financial Hardship / In-Service Withdrawal Form – To request from your employer a distribution from the plan due to a hardship event.
• Loan Request Form 1165 (E) Plan Employee – To request a distribution from the plan through a loan.
• Loan Balance Request Form: Plan Employee – Request to pay off your retirement plan loan.
• Enrollment and Changes for Catch Up Contributions Form – To ask your employer to deduct an additional deferral amount, if you are over 50 and the plan permits it.
• Direct Deposit Form – To authorize Banco Popular to deposit a plan distribution directly to your bank account.
• Declared Disaster Distribution Form - Perjury with Direct Deposit.
